Who are we looking for?
The Roving Clinical Care Coordinator plays a pivotal role in ensuring the delivery of high-quality, across a number of our homes. This position is responsible for stepping in during periods of leave or absence, ensuring that clinical services are maintained at the highest standards. The Roving Clinical Care Coordinator will assist in maintaining a positive and supportive work environment while mentoring and coaching staff to foster a culture of excellence in care delivery.
What can you expect to be doing?
- Provide clinical leadership and coordination across assigned homes, ensuring that person-centred care principles are adhered to.
- Conduct regular assessments of resident care to monitor progress, identifying potential risks and implementing appropriate interventions.
- Provide coaching, mentoring, and support to nursing and care staff to enhance clinical knowledge, skills, and performance.
- Act as a liaison between families, residents, and healthcare professionals, ensuring all parties are informed and involved in the care process.
- Work closely with senior leadership to identify, prioritise, and implement projects that enhance the overall efficiency and quality of clinical care across Residential Aged Care.
What do you need to bring?
- Demonstrated ability to manage complex care needs with previous senior clinical care experience.
- Strong leadership and mentoring abilities, with a focus on team development, motivation, and professional growth.
- Excellent written and verbal communication skills, with the ability to engage effectively with staff, residents, families, and external professionals.
- Ability to identify challenges and develop practical, person-centred solutions.
- Flexibility to work across multiple homes, adapting to different team dynamics and clinical needs.
- A strong commitment to promoting high-quality, evidence-based clinical care practices.
- Registered Nurse (RN) with current AHPRA registration.
- Experience in residential aged care or similar environments is highly desirable.
- Knowledge of relevant aged care regulations, standards, and guidelines (e.g., Aged Care Quality Standards)
